Treatment Transition

From the Treatment door to your door. Discharging out of residential care can be abrupt. You leave with a recovery plan and numbers but the truth is, most relapse within 2 weeks upon leaving treatment because the 24-hour care suddenly drops to 2-hour Aftercare groups a week with 12-step meetings to attend. Coming home is the time when you need sober support the most, where all the trappings of relapse are there waiting for you, where you drank in the first place. This is where your real life is.

Treatment Transition provides continuity needed to help you put your recovery plan into action with a smooth transition that helps you to maintain it. We understand what it takes to assist you back into sober life with your work and family to help you create your new normal.

Find your true purpose and transform your passions & dreams into reality.
